Iraqi and American military planners have their sights set on liberating Mosul from the self-proclaimed Islamic State (IS) – but as much as that city is suffering, it might not be the site of the country's largest humanitarian catastrophe.

That unfortunate distinction arguably belongs to Fallujah, which has been occupied by IS for more than two years.
